How to schedule a nutrition appointment.
- Log into your HappyHealthyMe.org Patient Portal account.

- Click ‘My Appointments‘ drop-down and then click ‘Schedule a New Appointment.’

- Choose a reason for your visit. In this case, it would be Nutrition Appointment.

- Choose the location you would like to visit.

- Select the day and time that is most convenient to you.

- Fill out any required fields and your contact information, and click Schedule Appointment.
