How to view lab results
- Log into your patient portal, HappyHealthyMe
- Click on the drop-down, My Health Record, and then click My Results.
- You will either see your most recent results or you can click Show all results.
How to view your referrals, after-visit summary, eye care prescription, vaccine records, and more.
- Log into your patient portal, HappyHealthyMe
- Click on the drop-down, My Health Record, and then click Clinical Notes
- Here you will find the referral form, eye care prescription, immunization records, and after-visit summary
How to direct book an appointment
- Log into your patient portal, HappyHealthyMe
- Click on My Appointments and then press Schedule a New Appointment
- Choose a reason for your visit
- Choose which location you would like to visit
- Select a date and time
- Describe any specific requests; how we should contact you, and press send me an email with my appointment details.
How to get your COVID-19 Results
- Log into your patient portal
- Click My Health Record drop down and then My Results
- Scroll down to the bottom of the page, where you will see Misc Labs
How to download the HappyHealthyMe App, HealtheLife
- Search on your app store, HealtheLife
- Once downloaded, it will prompt you to search for an organization. Search for Employee Benefit Funds
- You will then be directed to the HappyHealthyMe log in!
How to send a message to Patient Services/HIM/Care Team
- Log into your patient portal, HappyHealthyMe
- Click on My Messages, and Inbox
- Once your Inbox opens, press Send a message
- Once the message opens, in your * To box, you must enter two or more characters to prompt the selection you would like.
